 Intraoperative Approach



The mucosal incision is made in the vestibule. The periosteum is elevated to give good access and the distractor is placed in the desired position.

The microplates can be bent to ensure good contact to the cortical bone.

A single screw is placed on either side of the planned osteotomy cut.

After this the screws and the distractor are removed. The osteotomy of the buccal cortex as well as the osteotomies of the upper and lower border are now being performed between the screwholes.

The distractor can now be replaced and fixed with three screws on each plate.

It is recommended to use 3.5 mm or 5 mm screws in a child’s mandible.

Than a periosteal elevator or an osteotome is placed into the upper rim osteotomy and twisted until a greenstick fracture of the lingual cortex occurs.

Intraoperatively the correct function of the distractor is checked. The first distraction of 1-2 mm is performed immediately.

A distraction rate of 1 mm per day is recommended from the first postoperative day.

The distractor will be removed according to the treatment protocol taking into account the age of the patient, the actual lengthening and other therapeutic procedures.

The plates are carefully bent with a pair of bending pliers 25-486-13. Overbending of the plates has to be avoided as well as bending of all parts of the distractor. It is recommended to use the bending pliers 25-486-13 in combination with the distractor holding forceps 51-515-90 (see illustration). Caution: extreme care must be taken to bend only the plates of the distractor. Do not attempt to bend any portion of the distractor body.
